As a Ghoul, you unlock unique abilities like Feral and Glow. The Feral meter starts at 100% and decreases over time, but consuming Chems can boost it. Here’s what the Feral meter provides in Fallout 76:

Downsides of Being a Ghoul in Fallout 76
Becoming a Ghoul has its drawbacks. The “Leap of Faith” questline may strain relations with certain factions, which can complicate quests and gameplay. Additionally, your Charisma takes a hit, making social interactions less pleasant.
However, Fallout 76 offers a workaround. Jaye Vo, a new NPC in the “Leap of Faith” questline, provides disguises to help you navigate missions and avoid conflicts with hostile factions.
Is Becoming a Ghoul in Fallout 76 Worth It?
Despite the challenges, the Ghoul transformation in Fallout 76 offers compelling features that are hard to ignore. If you later prefer your human form, you can revert from the Character screen, though this prevents completing the “Leap of Faith” questline. You can transform again after finishing the questline for 1,000 Atoms.
Armed with this knowledge, head to the Savage Divide to unleash chaos in your new Ghoul form.
